The advice sounds simple enough. Eat less, move more. Create a calorie deficit. Be consistent. The math should work.

And for a lot of people, for a while, it does. Then it stops. The deficit that worked at first produces nothing—the scale stalls. The hunger gets harder to manage—the energy drops. The frustration builds. And eventually the conclusion — usually directed inward — is that something is wrong with you.

Here's what's actually wrong: the strategy stopped working because your nervous system perceived a threat. And a nervous system that perceives threat does not cooperate with weight loss. It does the opposite.

What Happens When You Cut Too Much

Caloric restriction is a physiological stressor. A mild, temporary deficit — the kind that comes from making better food choices without dramatically reducing volume — is something your body can adapt to without alarm. A significant, sustained deficit is something else entirely.

When your body receives consistently less energy than it needs to run all of its systems, it activates a threat response. Cortisol rises. Metabolism slows — not metaphorically, but measurably, as your body downregulates energy expenditure to match the reduced input. Thyroid function decreases. Muscle tissue begins to be broken down for energy because it's metabolically expensive to maintain. And fat storage — particularly around the abdomen, where cortisol drives preferential fat deposition — is actively protected.

This is your body doing exactly what it evolved to do in a famine. It cannot distinguish between intentional caloric restriction and actual food scarcity. The signal is the same. The response is the same.

The result is the maddening plateau that every chronic dieter recognizes: eating less than almost anyone around you, feeling worse than you did before you started, and making no progress.

The Cortisol Layer That Makes It Worse

For most of the patients I see, undereating isn't happening in isolation. It's happening in the context of a life that is already stressful — a nervous system that is already running a chronic low-grade threat response before the food restriction enters the picture.

Cortisol — the primary stress hormone — does several things relevant to weight and metabolism. It raises blood sugar in preparation for a threat response, which triggers an insulin response that drives fat storage when the blood sugar isn't used for actual physical activity. It preferentially deposits fat viscerally — around the organs and the midsection. It breaks down muscle tissue. It disrupts sleep, which affects the hunger hormones ghrelin and leptin in ways that increase appetite and reduce satiety signaling the following day. And it makes the already-stressed nervous system interpret a caloric deficit as one more threat to respond to.

Adding aggressive caloric restriction to a nervous system already running on cortisol doesn't create the conditions for weight loss. It stacks one stressor on top of an existing stress load and produces the metabolic equivalent of a system in full protection mode.

What Actually Works

This is not an argument for eating without intention or ignoring the role of food quality and quantity in body composition. It's an argument for a different starting point — one that addresses the nervous system state underneath the eating patterns rather than trying to override it with willpower and restriction.

Practically, this looks like:

Eating enough to signal safety. Your body needs to receive adequate fuel — particularly protein — to understand that it is not in a famine. For most women, this means eating more than their most recent diet suggested, not less. The short-term result may feel counterintuitive. The longer-term metabolic result is a system that stops hoarding.

Prioritizing protein at every meal. Protein is the most metabolically active macronutrient — it requires more energy to digest, it supports muscle maintenance, it produces the strongest satiety signal, and it keeps blood sugar more stable than carbohydrate-heavy meals. For stressed patients specifically, adequate protein is one of the most direct ways to support the metabolic function that cortisol is suppressing.

Addressing the nervous system directly. Food changes alone cannot fully resolve a cortisol-driven metabolic pattern. The nervous system has to receive consistent signals that the threat has passed — through sleep, through movement that isn't another stressor, through parasympathetic activation.
